By filling in this form and in relation to my child (“The Cricketer”), I understand and/or agree and/or undertake that my child will:

• attend matches in which the Zoo Lake Cricket Club (“the Club” or “ZLCC”) is engaged;
• participate in any matches in which he/she is selected to play for the Club;
• attend at any reasonable place for the purpose of and to participate in training and match preparation, failure of which will remain crucial to my child’s eligibility for selection in any team;
• play to the best of his/her skills and abilities at all times;
• conduct himself/herself on and off the field in a manner that is appropriate of a Central Gauteng Lions affiliated cricketer, a Club member and representative at all times; and
• refrain from acting in a manner that is abusive or belittling towards any other person or group on basis of their gender, race, disability, sexual orientation, age or status whether as sportsman or otherwise;
• have medical aid available as injuries are common in sport (the Club will not be held responsible for any injuries that occur on the field of play).
