Membership Application Form

By filling in this form and in relation to my child (“The Cricketer”), I understand and/or agree and/or undertake that my child will:

  • attend matches in which the Zoo Lake Cricket Club (“the Club” or “ZLCC”) is engaged;
  • participate in any matches in which he/she is selected to play for the Club;
  • attend at any reasonable place for the purpose of and to participate in training and match preparation, failure of which will remain crucial to my child’s eligibility for selection in any team;
  • play to the best of his/her skills and abilities at all times;
  • conduct himself/herself on and off the field in a manner that is appropriate of a Central Gauteng Lions affiliated cricketer, a Club member and representative at all times; and
  • refrain from acting in a manner that is abusive or belittling towards any other person or group on basis of their gender, race, disability, sexual orientation, age or status whether as sportsman or otherwise;
  • have medical aid available as injuries are common in sport (the Club will not be held responsible for any injuries that occur on the field of play).

Fees

For the 2023 calander year, the Club's Executive Committee have taken a decision that we will not raise a membership fee. The only costs payable will be a R 100-00 match fee (pay-as-you-play).
Do you as parent agree to make payment of this fee for all matches that your child will be selected for?

In the event that you would like to make an additional financial contribution towards club expenditure or enter into a sponsorship agreement with the club, please indicate so below. One of our Executive Committee members will then contact you to discuss your contribution.
Cricketer Indemnity

 I, the above named Parent/Guardian of the above-mentioned Cricketer:

  • fully understands and accepts that the playing and training of cricket at any Zoo Lake Cricket Club (“the Club”) coaching sessions or coaching festivals or matches (collectively “Cricket Performances”) is a high risk sporting activity. As such I the Parent/Guardian of the above-mentioned Cricketer, fully indemnify the Club as well as their coaches, employees, consultants or league organisers as a whole from and against any claims, demands, losses, costs, damages, action, suits or proceedings (including any attorney or legal fees) arising out of or resulting from injury during any Cricket Performance or any other Club activity participated in by the Cricketer from time to time. This indemnity includes and is also attributable to any bodily injury, sickness disease, disability, death, or any damage to tangible property.
  • consent that the Club can send the documents and information as provided to the Club by me to any third party to enable the Club to conduct its service offering in relation to the Cricketer;

With the below acceptance check box, I the undersigned Parent/Guardian fully subscribe and agree to the above and in relation to the Cricketer.
Protection of Personal Information

 Zoo Lake Cricket Club is regarded as a responsible party as defined in the Protection of Personal Information Act, 4 of 2013 (POPIA) and accordingly collects and processes your and your child's Personal Information (the "Personal Information"). In accordance with the provisions of the POPIA you hereby consent to the collection and processing of your and your child's Personal Information by ZLCC. In addition to consenting to the collection of your the Personal Information as discussed above, you further acknowledge that processing of the Personal Information may also occur in situations where processing:

  • is required by law or is necessary for a legitimate purpose;
  • is required to fulfil obligations imposed by a contract to which a ZLCC member may
  • be a party and/or will protect your or your child's interests.

You further acknowledge that the Personal Informatoin may be collected and preocessed for a variety of processes including but not limited to:

  • the accreditation and provisions of affiliations
  • registration of members with the club and other affiliated bodies
  • the provision of various member and affiliate related services
  • stakeholder relations management

You also consent to thereto that the club can keep the documents and information as mentioned above at the Club's offices, store it on the Club's server and keep it for as long as the Club needs to in terms of the Laws of the Republich of South Africa as long as the Club takes all reasonable steps to safeguard Personal Information and documentation and to destroy it when the Cricketer ceases to be a member of the Club.
